What are these and where can I get them?

Maya seems to possibly have some kind of infection in her nose wrinkle. We try to clean it regularly with baby wipes but perhaps not good enough. The wrinkle looks swollen and it is red underneath and smells pretty bad. Should I take her to the vet, will these Maleseb Pledgets help clear it up? Any suggestions are much appreciated.
